What this website collects
Most of this site collects nothing. You can read every page without creating an account, and we do not use advertising trackers, analytics scripts, or cookies of our own.
The one exception is the appointment request form. If you choose to use it, we receive what you type: your name, phone number, preferred office, best time to call, and an optional message. We ask you not to include medical details in that form.
How we use it
We use the form information for one purpose: to call you back and schedule a visit. We do not sell it, share it with advertisers, or add you to a mailing list.
Services this site relies on
- Cloudflare hosts the site and keeps standard server logs, such as IP address and pages requested, for security and reliability.
- Google Fonts provides the typefaces, which means your browser requests font files from Google.
- Google Maps provides the embedded maps on the office pages. Google may set its own cookies when a map loads.
- Web3Forms delivers appointment requests from the form to our office by email.
Each of these companies has its own privacy policy that governs what they do with the data they receive.
Your medical records
Nothing on this website is part of your medical record. Your records are protected under HIPAA, and the practice's Notice of Privacy Practices explains your rights, including how to request a copy of your records or ask for a correction. Ask for it at the front desk of either office, or call (936) 271-2222.
