Who we see
We are internal medicine physicians, which means we care for adults, generally 18 and older, as their primary care doctors. Many of our patients are 50 and up, and a large share are on Medicare. If you are looking for one doctor to manage your overall health, your ongoing conditions, and your referrals, that is what we do.
Insurance we accept
We participate in Medicare and the WellMed care network, and we accept Medicare Advantage, UnitedHealthcare, and most major commercial plans. Coverage varies by plan, so call us with your insurance card in hand and our staff will verify it before your visit. If we are not in your network, we will tell you up front.
How to schedule
Call (936) 271-2222, Monday through Friday, 9:00 AM to 4:00 PM. One number reaches both offices. Tell the staff you are a new patient, which office you prefer, and whether you have a doctor preference. If you would rather not call, send a request online and we will phone you back.
What to bring
- A photo ID and your insurance cards, including your Medicare card if you have one
- Every medicine you take, in the bottles if possible, including vitamins and supplements
- Records from your previous doctor, or their fax number so we can request them
- A list of your other doctors and any specialists you see
- Recent lab results, hospital discharge papers, or test reports if you have them
- Your glasses and hearing aids if you use them
- Your questions, written down. Visits go better when you do not have to remember them on the spot.
You are welcome to bring a family member or friend into the visit with you.
What the first visit is like
Plan on extra time for your first visit. The staff will take your paperwork and verify your insurance, and then your doctor will sit down with you to go through your history, your medicines, and whatever is on your mind. We may draw blood in the office the same day. You will leave with a plan and a follow-up date, not a rushed goodbye.
If you are on Medicare, your first visit can be scheduled as a Welcome to Medicare or Annual Wellness Visit, which is covered in full.
Transferring your records
Ask your previous office to fax your records to (936) 271-2228. If that is a hassle, we can request them for you after you sign a release at your first visit.
Our two offices
Choose whichever is closer. The Pine Forest office is just off I-45 near Shenandoah and The Woodlands. The Commercial Circle office is west of I-45 in central Conroe. Both see the same doctors and share your records.
Common questions
-
Are you accepting new patients?
Yes, at both offices. Call (936) 271-2222 and tell the staff you are a new patient. First visits are usually available within a week or two, and sooner if you have an urgent need.
-
Do I need a referral?
No. You can call us directly. If you have an HMO plan that requires you to name a primary care physician, our staff will help you with that step.
-
Can I see either doctor?
Yes. Dr. Farhat Khan and Dr. Perwaiz Kahn both see new patients at both offices. If you have a preference, say so when you call.
-
How do I transfer my records from my old doctor?
Ask your previous office to fax your records to (936) 271-2228, or bring a copy with you. We can also send a records request on your behalf once you sign a release at your first visit.
-
Do you see children?
No. We are internal medicine physicians, which means we care for adults, typically 18 and older.
